It didn’t look at all to me like Nagisa died. She was just very tired after giving birth. Tomoya saw her doze off and, knowing all about her vast collection of health problems, merely assumed that she died.

- To prevent any confusion, I think we should just say that Nagisa died, rather than saying Tomoya only assumed Nagisa's death. After all, her death is immediately confirmed in the next episode.
